This commit removes TARGET_INSN_START_EXTRA_WORDS and force all arch to
call the same version of tcg_gen_insn_start, with additional 0 arguments
if needed. Since all arch have a single call site (in translate.c), this
is as good documentation as having a single define.
The notable exception is target/arm, which has two different translate
files for 32/64 bits. Since it's the only one, we accept to have two
call sites for this.
As well, we update parameter type to use uint64_t instead of
target_ulong, so it can be called from common code.

The CPUClass::disas_set_info() handler is meant to initialize
the %disassemble_info structure; it shoudn't alter the CPU state.
Enforce the CPUState can not be modified by having the handler
take a const pointer.

Prefer the address_space_ld/st API over the legacy ld_phys()
because it allow checking for bus access fault.
Since we removed the last legacy uses of the legacy ldst_phys()
API, set the TARGET_NOT_USING_LEGACY_LDST_PHYS_API variable to
hide the legacy API to alpha binaries, avoiding further API uses
to creep in.

All these CPUAlphaState fields are of uint64_t type (except
the @fir[] array which uses float64, expanded to the same
type definition). Use the appropriate VMSTATE_UINT64() macro.
There is no functional change (the migration stream is not
modified), because the Alpha targets are only built as 64-bit:

CPUState::cpu_index is a target agnostic field, meant
for common code (i.e. accel/ and system/ folders).
Target specific code should use the CPUClass::get_arch_id()
helper, even if there is a 1:1 mapping.
In preparation of generic changes around CPU indexing,
introduce the whoami helper to access the generic
CPUState::cpu_index field.

Combine 3 different pointer returns into one structure return.
Include a cflags field in TCGTBCPUState, not filled in by
cpu_get_tb_cpu_state, but used by all callers. This fills
a hole in the structure and is useful in some subroutines.

The only place we really need to know the minimum is within
page-vary-target.c. Rename the target/arm TARGET_PAGE_BITS_MIN
to TARGET_PAGE_BITS_LEGACY to emphasize what it really means.
Move the assertions related to minimum page size as well.

Currently we handle flushing of output denormals in uncanon_normal
always before we deal with rounding. This works for architectures
that detect tininess before rounding, but is usually not the right
place when the architecture detects tininess after rounding. For
example, for x86 the SDM states that the MXCSR FTZ control bit causes
outputs to be flushed to zero "when it detects a floating-point
underflow condition". This means that we mustn't flush to zero if
the input is such that after rounding it is no longer tiny.
At least one of our guest architectures does underflow detection
after rounding but flushing of denormals before rounding (MIPS MSA);
this means we need to have a config knob for this that is separate
from our existing tininess_before_rounding setting.
Add an ftz_detection flag. For consistency with
tininess_before_rounding, we make it default to "detect ftz after
rounding"; this means that we need to explicitly set the flag to
"detect ftz before rounding" on every existing architecture that sets
flush_to_zero, so that this commit has no behaviour change.
(This means more code change here but for the long term a less
confusing API.)
For several architectures the current behaviour is either
definitely or possibly wrong; annotate those with TODO comments.
These architectures are definitely wrong (and should detect
ftz after rounding):
* x86
* Alpha
For these architectures the spec is unclear:
* MIPS (for non-MSA)
* RX
* SH4
PA-RISC makes ftz detection IMPDEF, but we aren't setting the
"tininess before rounding" setting that we ought to.

In do_cvttq() we set env->error_code with what is supposed to be a
set of FPCR exception bit values. However, if the set of float
exception flags we get back from softfloat for the conversion
includes a flag which is not one of the three we expect here
(invalid_cvti, invalid, inexact) then we will fall through the
if-ladder and set env->error_code to the unconverted softfloat
exception_flag value. This will then cause us to take a spurious
exception.
This is harmless now, but when we add new floating point exception
flags to softfloat it will cause problems. Add an else clause to the
if-ladder to make it ignore any float exception flags it doesn't care
about.
Specifically, without this fix, 'make check-tcg' will fail for Alpha
when the commit adding float_flag_input_denormal_used lands.
